As far as an Acknowledgment, this is going to be based on the 'custom' in your area.

I would start by looking in the Statutes for your State on how one aknowledges a deed.

If one were to get in a pinch, I would think a simple Notice stating some reference to the name on the Birth Certificate (exactly as spelled and styled) and any reference number on the Birth Certificate along with some writing stating the intent to acknowledge the gift, and then recorded at the County Records level would do the trick.

Remember, in law, intent (motive) is paramount.

Be creative. The path is narrow, but there is still room to move side to side.
